Medicaid Service Coordination

Established in March 2000, this unique program is designed to assist individuals of all ages (infants to seniors) in gaining access to necessary or desired services appropriate to the needs of the individual.
 
A Medicaid Service Coordinator (MSC) is the person who works directly for the individual and with the family to assist in the areas of referral and coordination of services.
 
The MSC assists with benefits entitlements, future care planning and waiver programs, is knowledgeable about community resources, medical services, home modifications, day programs, recreation, educational supports and advocacy, early intervention, residential and vocational services.
 
They also arrange for temporary relief such as respite, provide assistance in dealing with other agencies (social services and the school system).
 
We receive referrals through the schools, family members and other agencies.
 
The Medicaid Service Coordination department currently serves individuals in Schoharie, Albany, Schenectady, Otsego and Greene Counties.
 
We believe in promoting and supporting independence, an individual’s choice, and their rights to make informed choices.
 
The MSC department is committed to a person centered/person first philosophy.
This support service requires a MR/DD diagnosis and determination of eligibility through OMRDD, as well as Medicaid.
